You have just completed your Follow-Up Appointment as part of
your long term condition Yearly Health Check.
Do I have to answer this questionnaire?
No, you can opt out. However, it may be helpful for your GP
Surgery if you give feedback.
Do I have to answer all the questions?
No you don’t have to but if you do, it will be helpful to your GP surgery to have your feedback. Each question has space to explain the number you have given. This is optional but will help us understand what is working well and what could be improved.
Will my GP Surgery know what I said about them?
The questionnaire is anonymous (your name or other personal details will not be included).
What happens with my responses?
Your anonymous responses will be saved and reviewed by the GP Surgery before sharing with their Patient Participation Group and other NHS health partners, to help improve practice services as well as wider health services.
