To help us provide safe and effective care, we encourage all patients to ensure that their next of kin (emergency) contact details are recorded and kept up to date with the GP practice. Having a trusted family member, carer, or friend listed as your next of kin allows us to contact someone who can support you if an urgent situation arises and you are unable to communicate with us yourself.
Accurate next of kin information can be especially important during medical emergencies and unexpected hospital admissions. By providing and updating these details, you can help us ensure that the right person can be contacted quickly when needed.

We will NOT disclose any details of your medical records to your next of kin unless you have explicitly asked us to. All legal safeguards remain for your private medical information. Next of kin details are only used if we are unable contact you directly.
When we may need to contact your next of kin:
- You experience a medical emergency and are unable to communicate with healthcare professionals.
- We receive important or urgent information about your health and cannot reach you directly despite repeated attempts.
- There is a concern for your welfare, such as a serious health event, safeguarding issue, or unexpected hospital admission.
Please contact reception if you need to add or update your next of kin details (they will provide you with a short form to complete).
